has been the greatest satisfaction of my professional life, and I appreciate the trust you have placed

I am writing to let you know that I will be closing my medical practice, effective Wednesday, March
11th, 2026.
For a urology referral, please contact your primary care physician, your health insurance company,
local medical society, or the physician referral lines o f the local hospitals.
Beginning March 16th, 2026, all medical records from my practice will be transferred to Desert River Solutions, who will serve as the official custodian of records. If you would like a copy o f your records or need them transferred to a new provider, please begin the request process after that date.
In case of emergency, please proceed to the closest emergency room, and they will follow their
protocol and communicate with me.
